Impact Grant Program

We are humbled by our ability to invest over $2 million dollars over the next three years to provide significant impact to help strengthen local programs in essential services for the seven counties in northern New Mexico that we serve: Santa Fe, Rio Arriba, Los Alamos, Taos, Colfax, Mora and San Miguel.

The Mission and Overview of our Impact Grant program

At Anchorum, we are aware that our health system cannot support the wellness of our community in a silo. We have historically provided select direct multi-year grants to support the mission of CHRISTUS St. Vincent to ensure quality care for all patients, with a focus on children in our community.

When we announced our new brand and strategic direction in September 2018, we identified the need to support our nonprofit community, who provide critical services addressing the social determinants of health, by introducing a competitive grant process within our Impact Investment model. Management worked with our Board of Directors, stakeholders and local community leaders to help guide our strategy and vision based on the needs of the people of our community.

Our inaugural competitive Impact Grant cycle began in January 2019 and was conducted with intension and a thoughtful process over the past six months. We held Pre-Proposal Workshops for applicants so they would have a clear understanding of our four Strategic Priority Areas of Focus, Grant-Making Principles and Funding Strategies that were formed by holding a summit for our stakeholders, the Community Health Needs Assessment data conducted by CHRISTUS St. Vincent, along with local data on social determinants of health:

  • Senior Health and Wellness
  • Adult Behavioral Health
  • Educational Attainment
  • Housing

To further identify the scope and criteria where we would concentrate our funding, Anchorum adopted grant- making principles that provided the framework for both our applicants and Impact Grant Committee members -- so there was clear understanding and direction from both sides on the commitment of Anchorum St. Vincent to address the social, behavioral, physical environment, and healthcare delivery needs of our people and community. We set a minimum grant amount of $25,000 in order to provide tangible resources for sustainable results for the organizations selected through the process.

In order to be more inclusive of the people we serve, Anchorum made a commitment to fund organizations supporting the more rural communities in northern New Mexico. As such, the following counties were identified: Santa Fe, Rio Arriba, Los Alamos, Taos, Colfax, Mora and San Miguel.

Grant Making Principles

  • Impact Provides identifiable, meaningful and measurable positive impact on the priority community populations.
  • Collaboration Priority will be given to organizations who have (or will) collaborate with new and existing organizations to solve issues in priority areas.
  • Innovation We will fund organizations who have a proven track record of serving and meeting the needs of priority community populations as well as funding new programs and strategies (i.e., Approaches that are not “business as usual”).
  • Direct Connection We will fund organizations who are committed to working with Anchorum St. Vincent and CHRISTUS St. Vincent focused on the Community Health Needs Assessment and programs to help reduce Emergency Department utilization and Length of Stay and (if applicable), will take all referrals from CHRISTUS St. Vincent.
  • Place We will fund qualified 501(c)3 organizations in good standing in the City and County of Santa Fe New Mexico as well a six northern New Mexico counties.
  • Prevention and Intervention - We will fund programs focused on near-term intervention, as well as mid-term and long-term prevention.

When planning for our scope, we realized that not all our investments would have an immediate near-term effect. In fact, our priority areas of focus address a broader perspective, providing diverse approaches to wellness with long, mid and near-term results. After all, the health of a community ranges over an individual’s life span and is a reflection of the environment of care from birth to senior living – with quality of life measured in each stage of the journey.

Grantmaking Scope

When planning for our scope, we realized that not all our investments would have an immediate near-term effect. In fact, our priority areas of focus address a broader perspective, providing diverse approaches to wellness with long, mid and near-term results. After all, the health of a community ranges over an individual’s life span and is a reflection of the environment of care from birth to senior living – with quality of life measured in each stage of the journey.

  • Long-Term - Longer-term social determinants are in scope for Anchorum grant-making. Anchorum will commit 20%-25% of its annual grandmaking budget to these needs.
  • Mid-Term - Mid-Term social determinants of health and community needs that have a direct health and hospital impact, will be 30% to 45% of the annual grantmaking budget.
  • Near-Term - Near term addressed immediate needs, that have direct and urgent health and hospital impact, and will be 30% to 45% of the annual grantmaking budget.

Results of the Impact Grant Application Process

  • A total of 62 applications were received
  • A total of 23 Site Visits were conducted by our Impact Grant Committee
  • Awarded 18 Impact Grants for a total of $700,000 for 2019

These 18 Impact Grants significantly increases support across a wide range of needs--providing our underserved populations with shelter, food, compassionate care, education—and hope.

Our common goal: To improve and sustain the health and wellness of our community—now, and for future generations.

Casa Milagro – Stabilization and Case Support
($70,000)

“Ending Homelessness Through Fostering Communities of Belonging”

Casa Milagro is expanding their current capacity to serve the community by strengthening their support structure, and to bridge that expansion to include a collaborative housing project for a small cohort of the most economically and mentally challenged, chronically homeless senior women.

Grant supports infrastructure improvements, transportation, and staffing.

Coming Home Connection - Senior Home Care
($70,000)

“Aging with Dignity”

Volunteer and low-cost Senior Home Care and End-of-Life Care that connects trained volunteers with seniors in need to provide support, including home visits and assistance with basic tasks such as pet care, meal preparation, companionship, errands and more.  This organization provides safeguards so that the seniors in our community can age safely, in the comfort of their homes, with dignity.

Grant supports expansion of services by funding a volunteer coordinator position.

Kitchen Angels - Senior Meals
($25,000)

“Ensuring Nutrition for Homebound and Chronically Ill Individuals”

Provide free, nutritious meals to homebound and chronically ill individuals in the greater Santa Fe area who have no other means to achieve their nutrition needs.

Grant supports an additional 2,809 meals for Seniors.

“Home Repair From the Heart”

Improves the quality of life for low income individuals and families through complimentary small home repairs and upkeep for qualifying applicants.

Grant supports “aging in place” through home repairs for seniors in northern New Mexico.

Compassionate Touch Network - Mental Health Art Therapy and Outreach

“Breaking the Silence for northern New Mexico Adults and Adolescents: Reducing Stigma to Improve Community Mental Health.”

Compassionate Touch Network offers a nontraditional programs that address the public health challenge of mental illness and suicide through three core projects that incorporates mental health literacy and the power of sharing story through the arts and the written word, with the goal of reducing stigma, shame, and silence to promote positive community mental health outcomes:

Gerard’s House - Grief and Trauma Support

“Comprehensive Grief Support for Adults”

Gerard’s House offers both Spanish-language and English-language grief support prevention and intervention services, as well as Stepping Stones services for adults.  They provide comprehensive crisis response services to the community.

Santa Fe Dreamers Project – Counseling for Dreamers/Immigrants

“Santa Fe Dreamers Project In-house Counseling Pilot Program”

Santa Fe Dreamers Project will provide in-house counseling to include crisis stabilization, professional counseling, and case management services for immigrants and refugee clients who have been subject to trauma and/or violence.

Grant supports a full-time bi-lingual Counselor and Case Manager.

“Trauma Treatment Expansion Project”

Solace will provide additional clinical capacity to serve adult clients with culturally competent, evidence-based trauma treatment services designed to reduce anxiety, depression and other PTSD-related symptoms stemming from sexual violence and domestic abuse.

The Mountain Center – Addiction Intervention & Recovery Support

“Harm Reduction Program”

The Mountain Center provides syringe exchange and related services such as Naloxone distribution, HIV/HCV testing, therapeutic adventures, and food distribution in rural northern New Mexico, including Santa Fe and Rio Arriba counties.

Grant supports a Therapist and an IBP (Behavioral Health) Provider.

The Medical Magnet Program (MMP) at St. Michael’s High School will provide a specific and rigorous sequence and selection of health science classes and hands-on clinical mentorship to students in grades 9–12 grade who have specific interests in pursuing a medical career path in college and beyond.

“Sustainable Workforce College and Career Readiness and Mentoring Program”

Reading Quest will employ professionally trained tutors to tutor individual Santa Fe public-school children who are reading one or more years below grade level, and to help children who are having trouble connecting with  reading become strong, enthusiastic readers.

“STEM Scaffold Santa Fe at Capital High School”

The STEM Scaffold Santa Fe program at Capital High provides hands-on STEM education and includes mentorship by caring local STEM college students and STEM professionals who serve as role models through listening, trust building, and working together on meaningful, hands-on STEM projects with real-life application. Grant supports mentors and administrative expenses to facilitate the program at Capitol High School in the 2019-20 school year.

Interfaith Community Shelter Group - Homeless Medical Respite

“Medical Respite Care Bed Program”

The Interfaith Community Shelter's (ICS) Medical Respite Care Bed Program will reserve eight medical respite care beds for men and women experiencing homelessness in order to eliminate any barriers to discharge from CHRISTUS St. Vincent Regional Medical Center and to provide the individual who is homeless with a safe, clean, dignified environment in which to fully recuperate from an illness or hospitalization, all with the goal of preventing relapse and/or re-hospitalization.

Grant supports eight medical respite care beds for homeless men and women in need.

SFPS Adelante Program – Homeless/Home Insecurity Student Support

“School Liaison Project”

Adelante provides direct services to students and their families experiencing housing instability and homelessness. Adelante today identifies and serves more than 1,700 students and siblings experiencing homelessness or home insecurity every year by providing bi-lingual services and includes advocacy, tutoring, evening programs, case management and material aid -- such as school supplies, food distribution and clothing.

Grant supports a part-time Elementary Liaison.
